You may send a written request for your Police Accident Report to the following address (enclosing a $5.00 fee in the form of a money order or cashiers check payable to the Manning Police Department). Please indicate the Driver’s Name and Date of Accident, along with your contact information and return address. The report may also be returned to you by fax upon request; just include that number with your information.

Should your collision have taken place on the Interstate, you will need to obtain a copy of the Police Accident report by downloading this Request for Officer’s Report (SCDMV Form FR-50) from the South Carolina Department of Motor Vehicles. Simply fill out the form as completely as you can, and sign. Send 2 copies along with a $6.00 check or money order payable to SCDMV to the address below:
