How to Obtain Your Allendale Police Accident Report

The essential evidence included in a Police Accident Report is a compilation of the Investigating Officer’s findings. This report is filed electronically, and a request is needed to get a copy. The report will typically include notations listing contributors to the accident, as well as a drawing or image showing the point of impact and visible vehicle damages.

The Records Division of the Allendale Police Department has provided us with the following details and points of contact to assist in the acquisition of your Police Accident Report. You will find specific instructions within the In Person and By Mail sections.

There is a $5.00 fee for the Police Accident Report, and your ID should match the address on this report. If you are an uninvolved party, you must complete an FOIA form (see below).

At the time of your request, you may be asked to provide a Freedom of Information Act FOIA Request Form. If not available at the station or on its website, you may download Form G-639 and the instructions from U.S. Homeland Security. Uninvolved parties may only be allowed a redacted copy (at the discretion of the Records Officer).
