Involved parties in the accident may request a free copy of the Police Accident Report (uninvolved parties, including attorneys and insurance companies, must pay a fee of $5.00) by mailing a letter with your contact information and return address, driver’s license copy, the case #, and date of accident to the following address :

If the traffic accident took place on the Interstate, it will be under the Highway Patrol’s jurisdiction. You may obtain a copy of the Police Accident Report by downloading this Request for Officer’s Report (SCDMV Form FR-50) from the South Carolina Department of Motor Vehicles. Be certain to fill out the form with as much detail as possible and include your signature. Send 2 copies along with a check or money order payable to SCDMV for $6.00, to the address below:
